In these terms and conditions, “Studio Dre” means the company Studio Dre Pty Ltd consisting of Andre Dias da Silva and staff members employed by the company; and management means the management of Studio Dre Pty Ltd. Venue refers to the selected venue at which the fitness classes are held.
On payment of the required fees, I acknowledge that my child will be bound by the terms and conditions set out and referred to on this enrolment form.
Patrons must abide by the Venue’s conditions of entry, available to download or view at the venue.
Enrolments and bookings must be made at least 24 hours in advance. Enrolments will not be taken over the phone. Enrolment forms will only be accepted via email or in person. No enrolments will be accepted without payment.
Studio Dre must be informed of all allergies and medical conditions, if any, upon enrolment of a child. If your child has a medical management plan (severe asthma or anaphylaxis reaction), a copy must be provided to Studio Dre.
Studio Dre must be notified of any changes to a child’s condition that may affect their care whilst at the service.
Please observe the severity ratings as outlined and advise by ticking the respective box alongside the severity rating located on the enrolment form.
Mild: Non-life threatening reaction/condition, not requiring medication.
Moderate: Non-life threatening reaction/condition requiring medication. Please contact Studio Dre to discuss the appropriate action required.
Severe: Life threatening reaction/condition requiring medication. Please contact Studio Dre to discuss the appropriate action required.
Children enrolled in fitness classes must be accompanied by an adult to the class and collected at the termination of the class.
Alternative care arrangements must be made for siblings. Adults and siblings are not permitted to be within the fitness room during the class unless previously arranged with Studio Dre. Children must be supervised on arrival and exit to the student, always hold your child’s hand in the car park/common area outside our studio and head directly to/from your car. This area is not a playground.
At least 1 weeks notice is required if any changes to bookings are made.
Please provide Studio Dre with notice to any changes to your address, phone number, email address and child allergy / medical conditions.
No make-up lessons or refunds are available for missed classes.
Refunds may be provided in exceptional circumstances only, resulting in 3 or more continuous classes being missed, at the discretion of Studio Dre.
Medical conditions, resulting in missed classes, must be supported by a medical certificate.
All refund requests must be made in writing. Any refunds granted will incur a $20 administration fee.
Appropriate exercise attire and closed in training shoes must be worn at all times. School uniforms are not permitted. Classes may at times be conducted barefoot however appropriate footwear must be worn on arrival to the venue. Sand must be removed from footwear and clothing prior to entering the premises. Always wear shoes to and from the class, and ensure hands, feet and faces are clean before entering the gym.
All items found are placed in lost property for a period of one (1) month and if not claimed will be donated to a charity after this time. Please note that Studio Dre take no responsibility and bears no liability (in negligence or howsoever) for any lost or stolen items whilst participating in classes. Lost property enquiries are to be directed to a Studio Dre staff member.
I acknowledge that I may receive relevant communications and marketing materials from Studio Dre. Studio Dre will not distribute any personal details to any third party without prior permission.
I acknowledge my child or who is under my care and control may be at times photographed or filmed during the fitness class or associated performance. Photographs or film will be solely used for the purposes of Studio Dre advertisement. Please advise in writing if you do not wish for your child to appear in photos or videos.
Course dates are subject to cancellation due to insufficient bookings. If a course is cancelled, an alternative date/location will be provided where possible or a refund given if no alternative date is available.
Studio Dre reserves the right to annually review and implement new pricing structures.
Studio Dre reserves the right to refuse admission to any person without warning due to inappropriate behaviour. Reasons may include failure to comply with the terms and conditions or any reason that Studio Dre deems appropriate.
Should any interpretation on construction of these rules and regulations arise, then the decision of management in relation to that question shall be final and no claim for compensation or damages or refund may be made by the patron against Studio Dre.
Patrons voluntarily assume all risk of all damage and loss (including property damage), patronal injury (whether it be fatal or non-fatal), economic and consequential loss whatsoever and howsoever arising (including by negligence) at any Studio Dre fitness class, including damage or loss caused by the acts or omission of other patrons or employees or any other person or thing being present at the venue.
I hereby release Studio Dre and all employees thereof from and against all liability, in negligence or howsoever, arising from any injury, illness or death or loss of or damage to property occurring to or sustained by me or any child of mine or who is under my care and control whilst in, at or near the venue used for Studio Dre fitness classes.
If any child of mine or who is under my care and control shall be attending the fitness class and that child suffers death, illness or injury or loss of or damage to property then I shall indemnify Studio Dre against any and all claims, demands, action made or brought against Studio Dre by or on behalf of the said child in respect of any such death, illness, injury, loss or damage.
